Steps to sign up for FORMED.org:

1. Visit FORMED.org
2. Click SIGN UP
3. Select "I Belong to a Parish or Organization"
4. Enter the parish Zip Code and select St. Agnes and St. Lawrence Parishes, 40 St. Agnes Dr. Bloomsdale, MO, 63627
5. Enter your Name and Email - and you are in!

After you sign up once, you do not need to sign up again. When you want to access the platform in the future, just click SIGN IN. Also, be sure to installed the FORMED app on your mobile devices.
